PROGRESS MEETING MINUTES
DATE OF MEETING: Dec. 22, 2005

SUMMARY

  • Review and Approval of Minutes Previous Progress Meeting.

II. Review of Action Items from Previous Progress Meeting.

  • All Utilities are complete and out of the way.

III. Review of Work Progress Since Previous Meeting

  • Placed Pier Cap.
  • Installed Cofferdam Existing Pier #1.
  • Drilled and Blasted Existing Pier #1.
  • Concrete Finishing work on the New Pier.

Next meeting scheduled for Tuesday , 1/3/06, 9:30 AM @ engineering field office. New office is at 235 Bridgton Road on Route 302 in Fryeburg.

Reviewed project schedule of work. Project is still behind the original schedule. However The overall project still is on schedule to be completed by the required completion date.

CPM will provide a new Revised Schedule of work at the Next meeting.

Week of Jan. 9 th - Setting Deck Panels.

Week of Jan. 16th - Continued Removal of Existing Pier # 2.

Decking and Re-steel ; Span # 1.

  • Review of Status of Submittals and Long Lead Items

Shop Drawing for the Structural Steel has been submitted and approved.

Steel is 100 % Fabricated.

Shop Drawing for the T-Walls has been submitted; Drawings are approved.

T-Walls are 100 % Fabricated , Installed and Complete.

Traffic control plan(including boat traffic )has been submitted and approved.

Temporary Erosion Control Plan has been submitted and approved.

Temporary detour plan has been submitted and approved.

QC/QA Plan for concrete has been submitted. , Revised and resubmitted. 

There are new Mix designs for Class LP, A , and S Concrete. Class “S” and Class “A” Has been approved . Class “LP” Still needs to be approved. 

QC/QA Plan for paving has been submitted.

Another QC/QA Plan has been submitted from Pike and needs to be approved. 

Shop Drawing for the Bearings has been submitted and approved.

Bearings have been delivered , inspected and found to be as required through visual inspection. 

Shop Drawing for the Pile Tips and pile Splices has been submitted and approved and delivered. Piles also have been delivered, Installed and are complete.

Shop Drawing for the Pier Nosing has been submitted and Approved.

The Wave Analysis has been submitted and Approved. Item is complete.

Shop Drawing for the Deck Formwork has been submitted. Precast Panels have been fabricated and are 100 % complete.

Shop Drawings for the Steel Bridge Rail has been submitted and approved.

Bridge Rail has been delivered , inspected and found to be as required through visual inspection.

Contractor has passing tests for ASC-Gravel for the project.

  • Field Observations, Problems, Conflicts and Problem Resolutions

The project has no outstanding/pending claims.

George Weston from Weston’s Farm was in attendance at the Progre ss Meetng and raised some concerns on the new bridge. He has some pictures of the elevation of the river being just below the beams on the existing bridge. The new bridge design has the elevation of the new beams about 2 ft. lower than the existing. This would place the new beams closer to the elevation of the river and closer to any ice or debris that might come down the river.

More concerns raised at the meeting were the depth of the beams being a large area subjected to wind ice and po ssibly water and debris loading during any future floods.

Also the width of the pier seems to be narrow and future problems may occur if any debris stikes the side of the pier.

The Contractor is concerned with actually setting the steel beams. They need to be adequately braced until ALL of the diaphragms are in place and connected. These beams during erection will have to be handled very cautiously to avoid any toppling of them.

There is some gaps between the pier nose armor and the new concrete. This area needs to be patched before the cofferdam is flooded. An approved patching material the contractor will use is Emaco 415 which will work with our current conditions and temperatures.

Also before CPM will be allowed to pull any sheets Jim Pellerin from inland fisheries must be notified in case he wants to witne ss removal of the sheets.

TENTATIVE PLAN AND OPTIONS:

Need to Set Structural Steel.

Need to place deck

Putting Traffic over new structure.

Options: 1. Temporary membrane and paving.

  • Run traffic directly on the concrete, plan grade.

(which is not going to be allowed)

  • Place concrete high during deck placement and Remove the exce ss before final membrane and paving.

The Contractor will submit a proposal on their plan of running traffic over the new bridge.

Also to protect the temporary piers from damage the contractor will drive sheet piles in front of the temporary pier bents to protect them from any ice or debris coming down the river until traffic is on the new bridge.

Erosion control measures were inspected and everything was in order.

The Contractor will maintain a 4.2 meter traffic lane at all times as stated in the Plans and Specs. If the Contractor wants to decrease the width of this travel lane temporarily during the day the Contractor will provide flaggers at their own expense during this lane limitation. If any vehicle cannot get through the project due to the limitation of the lane width the Contractor will

need to move to allow passage of the vehicle.

Need prices from CPM for the Following items :

None at this time.

Need to complete payment for Disposal of Timber Piles .

Payment for Striping.

Payment for Geotextile.

The Contractor has installed the bouy system to channel the boat traffic to safe passage through the work area and is working effectively.

The Contractor will maintain a 4.2 meter traffic lane at all times as stated in the Plans and Specs. If the Contractor wants to decrease the width of this travel lane temporarily during the day the Contractor will provide flaggers at their own expense during this lane limitation. If any vehicle cannot get through the project due to the limitation of the lane width the Contractor will

need to move to allow passage of the vehicle.

Need prices from CPM for the Following items :

None at this time.

The Contractor however has submitted a leter detailing some delays caused by the heavy rain the past few weeks.

Terry Macgillivray from a local snowmobile rental place was at the meeting to express concerns accessing the trails through the bridge project. It was assured that a passable access will be maintained safely through the Bridge Project.

Erosion control measures were inspected and everything was in order.

Traffic lights have been adjusted from the micro-wave to a timed system to accommodate the field conditions. They are working good now.

This has been relocated to a center channel due to the very low water levels in the river.

The Weston’s Farm house has concerns of damage to their house due to vibration from the construction. CPM will have the blaster monitor the effects of blasting the existing piers.

A 3 ft shoulder has been striped where pedestrians can walk on the relocated bridge.

The Contractor will maintain a 4.2 meter traffic lane at all times as stated in the Plans and Specs. If the Contractor wants to decrease the width of this travel lane temporarily during the day the Contractor will provide flaggers at their own expense during this lane limitation. If any vehicle cannot get through the project due to the limitation of the lane width the Contractor will

need to move to allow passage of the vehicle.

PROGRESS MEETING MINUTES
DATE OF MEETING: August 29, 2005

ATTENDEES: Craig Hurd MDOT, Mike Tanerillo MDOT, Wade Kieth CPM Constructors

SUMMARY: Review and Approval of Minutes Previous Progress Meeting.

II. Review of Action Items from Previous Progress Meeting.

  • All Utilities are complete and out of the way.

III. Review of Work Progress Since Previous Meeting

  • Completed Driving Pile at both abutments.
  • Continued Excavation for Pier Seal.
  • Continued Prepping for removal of existing piers.

Next meeting scheduled for Tuesday , 9/13/05, 9:30 AM @ engineering field office. New office is at 235 Bridgton Road on Route 302 in Fryeburg.

  • Review of Work Planned Before Next Meeting
  • Complete Pier Excavation.
  • Begin Removal of existing piers.
  • Frame for Pier Piles.
  • Driving Pier Piles.
  • Begin Forming Abutments.
  • Schedule of Work Status

Reviewed project schedule of work. Project is on schedule.

Project Schedule has been reviewed and approved.

The schedule will be reviewed at the next scheduled meeting on 09/13/05.

Tentative long range schedule. Week of the 12 th - Place Pier Seal, Week of the 19 th - Pier Footing .

Week of the 26 th - Pier Shaft, Week of the 3 rd of October (Fair Week) Pier Cap.

  • Review of Status of Submittals and Long Lead Items

Shop Drawing for the Structural Steel has been submitted and approved.

Steel is 100 % Fabricated.

Shop Drawing for the T-Walls has been submitted; Drawings are approved.

T-Walls are 100 % Fabricated.

Traffic control plan(including boat traffic )has been submitted and approved.

Temporary Erosion Control Plan has been submitted and approved.

Temporary detour plan has been submitted and approved.

QC/QA Plan for concrete has been submitted. , Revised and resubmitted.

There are new Mix designs for Class LP, A , and S Concrete. These need to be approved .

QC/QA Plan for paving has been submitted.

Shop Drawing for the Bearings has been submitted and approved.

Bearings have been delivered , inspected and found to be as required through visual inspection.

Shop Drawing for the Pile Tips and pile Splices has been submitted and approved and delivered. Piles also have been delivered.

Shop Drawing for the Pier Nosing has been submitted and Approved.

The Wave Analysis has been submitted and Approved.

Shop Drawing for the Deck Formwork has been submitted. Precast Panels have been fabricated and are 100 % complete.

Shop Drawings for the Steel Bridge Rail has been submitted and approved.

Bridge Rail has been delivered , inspected and found to be as required through visual inspection.

Contractor has passing tests for ASC-Gravel for the project.

  • Field Observations, Problems, Conflicts and Problem Resolutions

The project has no outstanding/pending claims.

Erosion control measures were inspected and everything was in order.

Traffic lights have been adjusted from the micro-wave to a timed system to accommodate the field conditions. They are working good now.

The Contractor has installed the bouy system to channel the boat traffic to safe passage through the work area and is working effectively.

This has been relocated to a center channel due to the very low water levels in the river.

The Weston’s Farm house has concerns of damage to their house due to vibration from the construction. CPM will have the blaster monitor the effects of blasting the existing piers.

A 3 ft shoulder has been striped where pedestrians can walk on the relocated bridge.

The Contractor will maintain a 4.2 meter traffic lane at all times as stated in the Plans and Specs. If the Contractor wants to decrease the width of this travel lane temporarily during the day the Contractor will provide flaggers at their own expense during this lane limitation. If any vehicle cannot get through the project due to the limitation of the lane width the Contractor will

need to move to allow passage of the vehicle.

Need prices from CPM for the Following items : None at this time.

Also will look into disposal of Abutment timber piles.

  • Review of Future Action Items

Any authorized persons who take exception to any statement in these minutes must notify the Resident in writing within seven days from receipt of the minutes, stating in detail the comment, correction or omission. Otherwise, these minutes shall stand as written.

  • CMP has finally gotten in and set the new lines. The Phone Company needs to transfer their lines to complete utility work.

III. Review of Work Progress Since Previous Meeting

  • Completed Temporary Abutment .
  • Completed Temporary Pier Bents.
  • Completed Prepping Bridge for Relocation.

Next meeting scheduled for Tuesday , 7/15/05, 9:30 AM @ engineering field office.

BRIDGE RELOCATION IS SCHEDULED FOR JUNE 29 TH , FIRST THING IN THE MORNING. THE ROAD AND RIVER TRAFFIC WILL BE CLOSED FROM 6 AM UNTIL BRIDGE IS MOVED.

The Contractor will begin its final notification process today.

The schedule will be reviewed at the next scheduled meeting on 07/15/05.

Steel is 10 % Fabricated.

Shop Drawing for the T-Walls has been submitted; Drawings are approved.

Traffic control plan(including boat traffic )has been submitted and approved.

Temporary detour plan has been submitted and approved.

QC/QA Plan for concrete has been submitted. , Revised and resubmitted.

The Wave Analysis has been submitted and Approved.

Need to erect a sign tomorrow to inform people that Weston’s Farm is still open.

The Contractor has installed the bouy system to channel the boat traffic to safe passage through the work area and is working effectively.

This needs to be relocated to a center channel due to the very low water levels in the river.

There has been a problem with pedestrians jumping off the bridge into the construction area, the Town Police have been notified.

The Weston’s Farm house has concerns of damage to their house due to vibration from the construction. CPM will address this issue after the bridge is moved.

When the bridge is relocated their will be a 3 ft shoulder striped where pedestrians can walk.

We will try to accommodate canoe traffic around the construction area during the bridge relocation.(river will be closed at that time to boat/canoe traffic). Some options will be to have a temporary portage during this time. Others are to put in at the beach below the closure. All Canoe rental places upstream of the closure will be notified of the closure well in advance.

The Contractor will maintain a 4.2 traffic lane at all times as stated in the Plans and Specs. If the Contractor wants to decrease the width of this travel lane temporarily during the day the Contractor will provide flaggers at their own expense during this lane limitation. If any vehicle cannot get through the project due to the limitation of the lane width the Contractor will need to move to allow passage of the vehicle.
