Reviewed project schedule. CPM provided a new Revised Schedule of work & is Approved as submitted. Project is on schedule.
Shop Drawing for the Structural Steel has been submitted and approved. Steel is 100 % Fabricated, Installed and Complete. Shop Drawing for the T-Walls has been submitted; Drawings are approved. T-Walls are 100 % Fabricated , Installed and Complete. Traffic control plan(including boat traffic )has been submitted and approved. Temporary Erosion Control Plan has been submitted and approved. Temporary detour plan has been submitted and approved , Detour is in Place. QC/QA Plan for concrete has been submitted. , and approved. There are new Mix designs for Cla ss LP, A , and S Concrete. Cla ss “S” and Cla ss “A” Has been approved . Cla ss “LP” has been submitted and is now Approved. QC/QA Plan for paving has been submitted. Another QC/QA Plan has been submitted from Pike and needs to be approved. Shop Drawing for the Bearings has been submitted and approved. Bearings have been delivered , inspected and found to be as required through visual inspection , Bearings have been installed and are complete. Shop Drawing for the Pile Tips and pile Splices has been submitted and approved and delivered. Piles also have been delivered, Installed and are complete. Shop Drawing for the Pier Nosing has been submitted and Approved, Installed and is complete. The Wave Analysis has been submitted and Approved. Item is complete. Shop Drawing for the Deck Formwork has been submitted. Precast Panels have been fabricated and are 100 % complete. Shop Drawings for the Steel Bridge Rail has been submitted and approved. Bridge Rail has been delivered , inspected and found to be as required through visual inspection. Contractor has passing tests for ASC-Gravel for the project
The project has no outstanding/pending claims. The Contractor will need to complete removal of existing Pier #’s 1 & 2. There is still concrete in that area that will have to be removed in the Spring/Summer. CPM will propose a containment plan when removal is planned. There is some gaps between the pier nose armor and the new concrete. This area needs to be dewatered and patched before pier is complete and accepted. Steel nosing plates also need to be cut flush at the seal concrete elevation. Erosion control measures were inspected and everything was in order. The Contractor will maintain a 4.2 meter traffic lane at all times as stated in the Plans and Specs. If the Contractor wants to decrease the width of this travel lane temporarily during the day the Contractor will provide flaggers at their own expense during this lane limitation. If any vehicle cannot get through the project due to the limitation of the lane width the Contractor will need to move to allow passage of the vehicle. Need prices from CPM for the Following items : None at this time Need letter from CPM Constructors / Coleman Concrete stating the cement used on the deck placement was the type stated in the plans. I ssues from the Town of Fryeburg : There is an exposed water pipe on the downstream side of the bridge. This is a Fryeburg Water Company Main. This has been uncovered during the Winter by the changing river bottom. The MDOT will take some measurements of the pipe and will monitor any changes in the river bed around the pipe.
Any authorized persons who take exception to any statement in these minutes must notify the Resident in writing within seven days from receipt of the minutes, stating in detail the comment, correction or omission. This will only be done if ice begins to form in the river and becomes an i ssue. Steel nosing plates also need to be cut flush at the seal concrete elevation. If the Contractor wants to decrease the width of this travel lane temporarily during the day the Contractor will provide flaggers at their own expense during this lane limitation. If any vehicle cannot get through the project due to the limitation of the lane width the Contractor will need to move to allow passage of the vehicle. Need prices from CPM for the Following items : Need Price invoice for Pier Nosing .. Need invoice to complete payment for Disposal of Timber Piles . CPM will submit a time extension due to delays from flooding and from delays from jacking the bridge.

The project has no outstanding/pending claims. The Contractor is concerned with actually setting the steel beams. They need to be adequately braced until ALL of the diaphragms are in place and connected. These beams during erection will have to be handled very cautiously to avoid any toppling of them. There is some gaps between the pier nose armor and the new concrete. This area needs to be dewatered and patched before pier is complete. TENTATIVE PLAN AND OPTIONS: Need to Set Structural Steel. Need to place deck Putting Traffic over new structure. Options: 1. Temporary membrane and paving.
(which is not going to be allowed)
